When should you replace the catylitic convertor? I have had problems in the past passing emmissions. The truck has been running rich for a long time.
I have fixed the rich condition, should I replace the convertor?

Do the emission test,or have a convertor effeciency test done.That will tell you exactly how well it's working.
Heat is also a sign the catalyst is still functional.After a good hard drive,the cat should be quite hot,much hotter than the rest of the exhaust system.

Can back pressure be checked to see if the converter is plugged?

Yes,you can check backpressure too,but i would only bother if the truck exhibits a driveability concern that would indicate a plugged cat.
Backpressure does not show convertor effeciency though.
